Clinical Study of Mahanimba Kwatha In Ghridhrasi (Sciatica)

Abstract:- Backache problems affect not only the social and economic position of the individual and his family, it leads to the draining of national resource due to the work hours lost , resulting in to diminished production.Ghridhrasi is one of leading Vatavyadhi causing backache. Mahanimba is a commonly available drug & Acharyas described it as uttam Ghridhrasihar.The details of the drug will be discussed in paper.


Introduction: – Vata vyadhi is one of the top most chronic problems of human beings. Acharya charak has mentioned “Nanatmaj vyadhi” of Vata, Pitta, Kapha, but separate chapter has been contributed to only Vata vyadhi. Acharya Vagbhat also considered Vata vyadhi as Maharog . Ghridhrasi is one of the Vata vyadhi mentioned by Charaka in the 80 types “Nanatmaj Vata vyadhi”.( 11) The most common disorder which affect the movement of the legs, particularly in the most productive period of life -i.e.30 to 50 years- is backache problems-a thirds of the patients turn in to Ghridhrasi.

Ghridhrasi is a condition where disfunction of vata affect ghridhrasinadi characterized by stambhaka (stiffness), ruk(pain), toda(paining sensation). Stambhaka starting from sphik,kati & prishtha radiating down the posterior border of the thigh, janu ,& outer border of jangha, pada & angulees , with stiffness & impediment of lifting of thigh.

The etiological factors which are mentioned for Vata vyadhi in general are deemed to be the etiological factors for Ghridhrasi also as there is no specific description of etiological factors for the Ghridhasi in Ayurvedic texts. However Ghridhasi has been classified into two groups: 1.Vataj 2. VataKaphaj.

Hence the factors which cause variation of kapha also have some role in the pathogenesis of Ghridhrasi of kapha origin associated with vata. The ancient Acharyas of Ayurvedic treatises & sanghra granthas made many pertinent contributions in the context of Gridhrasi chikitsa. Role of Mahanimba in Gridhrasi has been mentioned by Acharya Shodhal, Sharangdhar, Bhavamishra & Yogratnakar.

Aims & Objectives:-

To study the role of Mahanimba kwatha in Gridhrasi.

Materials & Methods:-


Drug : Mahanimba kwatha Dose: 25 ml.

Sevan kala: Prabhakala & Sayankala

Rout of administration: Oral


Latin name:- Melia azedarach

Family :- Meliaceae

Rasa :- Tikta,Katu.

Veerya :- Sheeta.

Vipaka :- Katu.

Guna :- Laghu, Ruksha.

40 patients were selected by simple random sampling method between the age group 15-60 yrs.

Duration of treatment:- 30 days

Follow up :- After 15 days

Subjective criteria

A) Inclusion criteria

1) Referred pain from sphika to pada

2) Stambha

3) Spandan

4) Tandra

5) Gaurav

6) Arochaka

7) Mukhapraseka

8) Dehasy avakrata


B) Exclusion criteria:

1) Diabetic neuropathy

2) Leprosy-peripheral neuropathy

3) Pelvic inflammatory disease

4) Nerve compression due to fracture

5) Neoplasm causing nerve compression

6) Patient advised surgery


Objective criteria

Straight leg rising test {SLRT}

Result assessed as:

l) Uttam upashya :- 75% symptoms subsided.

2) Madham upashya:- 50% symptoms subsided.

3) Alpaupashya :- More than 25% symptoms subsided.

4) Anupashya :- Less than 25% symptoms subsided.












Conclusion:- In ghridhrasi, kandara dushti mentioned by Sushruta .Kandara known as an updhatu of Rakta. According to modern concept, Sciatica nerve compression is due to prolapse of intervertebral disc or space reduction. Mahanimba acts on Rakta, Asthi, Majjavaha strotasa and correcting the kandaradusti and nurities.

57.5% patient get madhyam upshaya. Out of 10 symptoms, 7 subsided after 30 days by use of Mahanimba kwatha. More relief got in Vatakaphaj ghridhrasi symptoms.
