Effect of Fig leaves juice external application in Shwitra (Vitiligo)

Introduction:Ayurveda emphasizes on maintenance of positive health and alleviation of the diseases engulfing human kind, among which skin diseases are one, which are visible and agonizing the individual psycho-somatically. Vitiligo is such a common chronic and progressive skin disease characterized by the lack of melanin pigments producing skin patches with sharp and often hyper pigmented edges.In Ayurveda, Shwitra has been listed to be the worst amongst Kustha (Disorder of skin) to cause disfigurement of the body.

Case Presentation:A 54 years old, diagnosed case of Shwitra (Vitiligo) female patient attended the OPD (Outdoor Patient Department) of Panchakarma and complained with white patches at dorsum surface of her both forearms since 6 months and they gradually increased. No any relevant family history, any drug history for this or any systemic illnesses were found. Firstly she got counselled for the disease management and then treatment plan had been given to her.

VITILIGO
Absence of Melanin or Destruction of Melanocytes causes Vitiligo

Treatment Plan:Fig (Ficus carica) leaves juice: External application followed by morning sun-exposure about 20-25 min.

Time of Administration: Morning

Duration: 6 weeks or up to the formation of blister on the skin.

Results and Observations:Scoring before treatment and after treatment was noted. 74.54% relief was found in point counting method, 56.25% relief was found in VASI score while 46.67% relief was found in VETI score.Routine Haematological and Biochemical investigations done before and after treatment but no any major difference found in them except ESR (Erythrocyte Sedimentation Rate) increased after treatment.

Discussion:Fig (Ficus carica Linn.) leaf has been reported to contain furocoumarins including psoralens. Fig leaves are used for Vitiligo. Thus it is effective in Shwitra externally. The juice of fig leaves has long been used to treat Vitiligo. The furocoumarins psoralen, bergapten and the coumarins umbelliferone, 4′, 5’-dihydropsoralen and marmesin allergens are present mostly in leaves. They create photosensitivity while applied over skin (White patches) on the direct interaction with Ultraviolet rays of sunlight and skin becomes red and after that becomes normal in colour. Sometime it creates blisters due to severe photosensitivity.

Conclusion:Fig leaves are used for local application for Shwitra (Vitiligo) in Indian traditional medicines. External application of Fig leaves juice are the alternate and effective management for local application concluded as effective management in Vitiligo.
